over 18 years old and have a weakened immune system due to a health condition or medications • over 70 years old (regardless of vaccination status) • over 60 years old with fewer than three vaccine doses • over 18 years old with fewer than three vaccine doses and at least one of these risk conditions: • obesity • diabetes • heart disease • hypertension • congestive heart failure • chronic respiratory disease (including cystic fibrosis) • cerebral palsy • intellectual or developmental disabilities • sickle cell disease • moderate or severe kidney disease • moderate or severe liver disease • pregnant and unvaccinated

Our waiting room will be a COVID SYMPTOM FREE ZONE (please stay home if you are suspected for covid and/or covid+).
We ask that when you arrive, if you have ANY respiratory symptoms/flu-like symptoms, TELL RECEPTION STAFF RIGHT AWAY. You will be accommodated.

To contain the spread of COVID 19, until further notice we will:
CONTINUE to offer essential services, as usual. This includes urgent care appointments, routine prenatal care, and newborn visits/pediatric immunizations.
OFFER TELEPHONE/VIDEO appointments with your provider for timely, but less urgent concerns. This may include prescription renewals, blood pressure/diabetes checks, and new, less urgent concerns
DEFER any new bookings for some non-urgent concerns. This may include visits such as: periodic health exams (“physicals”), well child visits, pap tests, ear syringing, new patient appointments.
The use of email is intended for non urgent matters. All emails are approximately answered in 3-4 business days. Do not send any urgent matters using this email. If you need urgent attention, please call the clinic during office hours, call 911 or go to your nearest ER.
Please note that turn around time for faxed prescription renewals is 1 week.
For medical and insurance forms, please allow at minimum 2 weeks for completion.
